    Individualized dosing algorithms for tacrolimus in kidney transplant recipients:current status and unmet needs

    Introduction: Tacrolimus is a potent immunosuppressive drug with many side effects including nephrotoxicity and post-transplant diabetes mellitus. To limit its toxicity, therapeutic drug monitoring (TDM) is performed. However, tacrolimus’ pharmacokinetics are highly variable within and between individuals, which complicates their clinical management. Despite TDM, many kidney transplant recipients will experience under- or overexposure to tacrolimus. Therefore, dosing algorithms have been developed to limit the time a patient is exposed to off-target concentrations. Areas Covered: Tacrolimus starting dose algorithms and models for follow-up doses developed and/or tested since 2015, encompassing both adult and pediatric populations. Literature was searched in different databases, i.e. Embase, PubMed, Web of Science, Cochrane Register, and Google Scholar, from inception to February 2023 Expert Opinion: Many algorithms have been developed, but few have been prospectively evaluated. These performed better than bodyweight-based starting doses, regarding the time a patient is exposed to off-target tacrolimus concentrations. No benefit in reduced tacrolimus toxicity has yet been observed. Most algorithms were developed from small datasets, contained only a few tacrolimus concentrations per person, and were not externally validated. Moreover, other matrices should be considered which might better correlate with tacrolimus toxicity than the whole-blood concentration, e.g. unbound plasma or intra-lymphocytic tacrolimus concentrations.</p

    Viscosity and fission time scale of^{156}Dy

    In the fusion-fission reaction Ar-40+Cd-116-->Dy-156-->fission, performed at beam energies E(b) = 216 MeV and 238 MeV, gamma rays were measured in coincidence with fission fragments. The gamma-ray spectra are interpreted using a modified version of the statistical-model code CASCADE. From a comparison of the experimental and calculated spectra it is deduced that the nuclear viscosity is in the range 0.01 <gamma <4. The extracted fission time scale is of the order of 10(-19) s

    Exploring social patterns of participation in university-entrance level mathematics in England

    In recent years in England there has been considerable attention given to a range of apparent crises in mathematics education, one of which has been the long term decline of participation in university-entrance level (Advanced or A) mathematics. Given the negative impact upon mathematics participation of Curriculum 2000, together with the government’s emphasis on Science Technology Engineering and Mathematics (STEM) subjects, the political intent to increase participation in Advanced level mathematics is clear. This paper uses the National Pupil Database (NPD) to develop a descriptive statistical account of how completion of Advanced level mathematics varies along the social axes of SES, ethnicity and gender. The process of working with the NPD is discussed in some depth in order to clarify the processes involved in this type of quantitative analysis and to illustrate how this analysis can be used to raise questions about who is doing what mathematics in the post-16 age-range

    Compilation of Giant Electric Dipole Resonances Built on Excited States

    Giant Electric Dipole Resonance (GDR) parameters for gamma decay to excited states with finite spin and temperature are compiled. Over 100 original works have been reviewed and from some 70 of which more than 300 parameter sets of hot GDR parameters for different isotopes, excitation energies, and spin regions have been extracted. All parameter sets have been brought onto a common footing by calculating the equivalent Lorentzian parameters. The current compilation is complementary to an earlier compilation by Samuel S. Dietrich and Barry L. Berman (At. Data Nucl. Data Tables 38(1988)199-338) on ground-state photo-neutron and photo-absorption cross sections and their Lorentzian parameters. A comparison of the two may help shed light on the evolution of GDR parameters with temperature and spin. The present compilation is current as of January 2006.Comment: 31 pages including 1 tabl
